Amlodipine: a standard of care in the treatment of hypertension

Amlodipine is an active ingredient belonging to the class of dihydropyridine calcium channel blockers, used primarily for the treatment of hypertension and certain forms of angina pectoris. Because of its efficacy, long duration of action, and favorable tolerability profile, it continues to be a standard-of-care treatment in clinical practice.

How Amlodipine works and what it is used for 

Amlodipine works by blocking L-type calcium channels, which are found primarily in the smooth muscle of the arteries. This mechanism promotes the relaxation of blood vessels, reduces peripheral resistance, and helps lower blood pressure, thereby reducing the workload on the heart.

Thanks to this mechanism of action, amlodipine is used to treat:

  • hypertension, a condition characterized by a persistent increase in blood pressure, recognized as one of the main risk factors for cardiovascular, cerebrovascular, and renal diseases;
  • chronic stable angina, a form of chest pain or discomfort caused by reduced blood and oxygen supply to the heart muscle, which generally occurs during physical exertion or emotional stress;
  • vasospastic angina (or Prinzmetal’s angina), a form of angina caused by a transient spasm of the coronary arteries, which can also occur at rest.

Why it is an essential active ingredient

The high prevalence of hypertension – which, according to the World Health Organization, affects about one in three adults worldwide – makes it necessary to have effective and well-established active ingredients available for its treatment. Among these, Amlodipine plays a fundamental role: included in the WHO Model List of Essential Medicines, it is recognized by the WHO as one of the essential medicines needed to meet the primary health needs of the global population.

Its importance is also linked to its pharmacological characteristics. Thanks to a long half-life (30–50 hours), Amlodipine ensures prolonged blood pressure control and can generally be administered as a single daily dose, promoting treatment adherence. Furthermore, its versatility has enabled the development of numerous pharmaceutical products, both on its own and in combination with other antihypertensive drugs, making it possible to address a variety of therapeutic needs.
